Remove KPixmapEffect references. Currently the mouseover effect is disabled,

until I find a suitable effect to use instead of KPixmapEffect::intensity().
Fix another off-by-one rendering glitch in the buttons.

Fix this up so I can drag things around properly, and have

them get to their destination without getting misplaced or a
crash.
In short, keeping iterators to a changing QList is a Bad Idea(tm),
so I did the simple thing can used indices.
